Take the 4 train to Burnside Avenue. Walk 4 blocks east on Burnside, turn south and walk down the west side of the Grand Concourse to 2021.
Take the D train to Tremont Avenue. Walk 1 1/4 blocks north on the west side of the Grand Concourse to 2021.

Bus
Bx1, Bx2, Bx40, and Bx42 buses all stop at Burnside and the Grand Concourse. Bx36 bus stops at Tremont Avenue and the Grand Concourse. Follow the above instructions for walking to 2021.

Union Community Health Center

Subway
Take the D train to Fordham Road and the Grand Concourse. Walk one block south on the Grand Concourse, turn east onto 188th Street. Union Community Health Center is between Valentine and Tiebout Avenues, on the south side of the street.

Bx1, Bx2, Bx12, and Bx22 buses all stop at Fordham Road and the Grand Concourse. Follow the above instructions for walking from that intersection to the Union Community Health Center.

Bx34 bus stops at Fordham Road and Valentine Avenue. Walk one block south on Valentine to East 188th Street, turn east and continue to 260 East 188th.

2250 Ryer Avenue

Subway
Take the D train to 182 nd Street and the Grand Concourse. Walk 1 block east to Ryer and turn north. 2250 is between 182 nd and 183 rd Streets, on the east side of the street.

LOCAL Bx1 and Bx2 buses stop at 182 nd Street and the Grand Concourse. Follow the above instructions for walking from that intersection to 2250 Ryer.

817 East 180th Street

Subway
Take the 2 or 5 train to Tremont Avenue. Walk west on Tremont Avenue 4 blocks to Southern Boulevard. Walk north 3 blocks to the intersection of Southern Boulevard and 180th Street. 817 East 180th is at the southwest corner of the intersection.
